Goals
Preserve, protect and improve the health of individuals, families and communities through activities that improve the environment, encourage lifestyle and behavior change and assure access to health care.
What We Do
We help families and individuals throughout the life span in their homes and in the community. Field public health nurses focus on accomplishing their goals through:
- assessment of the client needs,
- identification of problems,
- establishing a plan collaboratively with the client, and
- facilitating interventions tailored to the established plan.
Public Health nurses inform, educate and empower people about health issues, and refer through community links to needed care.
We take referrals from many sources throughout the community, including physicians, social workers and concerned members of the general public. Field Public Health Nurses also reach out to anyone who might benefit from an intervention or service. The focus is on all people in our communities, not just those who request help.
