Affidavit of Late Notice If you did not receive a notice of assessment at least 15 days before the deadline to file an application, you may file an application within 60 days of the mailing of the tax bill, accompanied by an affidavit declaring under penalty of perjury that the notice of assessment was not timely received.

Application for Changed Assessment - Fillable/ADA Compliant Applications are available for the Regular Assessment periods from July 2 through September 15th. If September 15th falls on Saturday, Sunday, or a legal holiday, an application that is mailed and postmarked on the next business day shall be deemed to have been filed within the specified time period. Supplemental or Escaped Assessments applications may be filed within 60 days after the mailing date of the supplemental notice or notice of proposed escaped assessment. Applications for Calamity Assessments must be filed within six months of the mailing of the notice. The application obtained on-line must be printed on 8½"x14" paper and be submitted with an original signature to the Clerk of the Board of Supervisor's office.
Application for Electronic Submission of Assessment Appeal Applications In order for you to be able to submit your Assessment Appeals online without the need to provide signed, hard copies (E-File), you must complete this form and submit it to the Clerk of the Assessment Appeals Board in order for them to generate a PIN for your account. You may either submit the form in person or by mail. If you submit in person, bring your Driver’s License or California ID with you. If you submit via mail, you must have the form notarized, and include a copy of your Driver’s License or California ID.
